Musnad Ahmad ibn Hanbal 5602
It is narrated on the authority of Sayyiduna Ibn Umar (may Allah be pleased with him and his father) that at the time of the Prophet (peace and blessings of Allah be upon him), the words of the Adhan used to be pronounced twice each, and the words of the Iqamah once each. However, the Mu'adhdhin used to say "Qad qamatis-salah (The prayer has commenced)" twice.

سیدنا ابن عمر رضی اللہ عنہما سے مروی ہے کہ نبی ﷺ کے دور باسعادت میں اذان کے کلمات دو دو مرتبہ اور اقامت کے کلمات ایک ایک مرتبہ کہے جاتے البتہ مؤذن «قد قامت الصلوة» دو مرتبہ ہی کہتا تھا۔
Sayyidna Ibn Umar razi Allah unhuma se marvi hai ki Nabi (صلى الله عليه وآله وسلم) ke daur basaad'at mein azaan ke kalimat do do martaba aur iqamat ke kalimat ek ek martaba kahe jate albatta mo'azzin «qad qamat issalat» do martaba hi kehta tha.
حَدَّثَنَا عَبْدُ الرَّحْمَنِ بْنُ مَهْدِيٍّ ، حَدَّثَنَا شُعْبَةُ ، عَنْ أَبِي جَعْفَرٍ ، سَمِعْتُ أَبَا الْمُثَنَّى يُحَدِّثُ، عَنِ ابْنِ عُمَرَ ، قَالَ:" كَانَ الْأَذَانُ عَلَى عَهْدِ رَسُولِ اللَّهِ صَلَّى اللَّهُ عَلَيْهِ وَسَلَّمَ مَثْنَى مَثْنَى، وَالْإِقَامَةُ وَاحِدَةً، غَيْرَ أَنَّ الْمُؤَذِّنَ كَانَ إِذَا قَالَ: قَدْ قَامَتْ الصَّلَاةُ، قَالَ: قَدْ قَامَتْ الصَّلَاةُ مَرَّتَيْنِ".
